Abstract: A method for enabling remote self service retrieval of medical information for an individual is disclosed. The method includes steps of storing personal information and medical information in different databases. Different encryption keys are used to isolate the types of information. Upon receipt of a request from a health care provider, the authenticity of the request is verified. At a central location, using the identification of the individual, the medical information for that individual is retrieved and provided to the health care entity treating the individual.
