Abstract: A system and method of collecting feedback from a user is disclosed. The system and method may include providing a server capable of storing a survey and generating a unique link to access the survey on the server, the server further capable of sending the unique link to at least one of the user and a callback clerk. The system and method may also include completing the survey by the user and returning the completed survey to the server, determining by the server if any actions in the completed survey have been triggered and gathering the responses in the survey into at least one report by the server.
