The test kit includes a test strip, which in turn includes an embedded data object having a unique identification of the test strip. The test strip further includes a test sample collection patch configured to receive a test sample from the individual. The application is configured to be resident on a host machine. The application includes machine instructions that when executed by a processor of the host machine, control the host machine to capture the content of the embedded data object, control the host machine to capture an indication of the test sample, and control the host machine to transmit the content and the indication of the test sample to a processing facility of the health safety and status system.
