The patient profile comprises, for example, demographic information, geographic location, clinical characteristics, patient preferences, a patient need level, a category to which the patient is assigned, and a priority level associated with the patient. A set of characteristics associated with a group of care managers is accessed where the set of characteristics include clinical expertise of the care manager, geographic location of the care manager, workload capacity of the care manager, and the like. Using the patient profile and the set of characteristics associated with the group of care managers, the patient is aligned with a care manager well-suited to help the patient achieve care management goals.

    Abstract: A dynamic licensing system permits facility administrators to quickly enable a patient monitoring device to monitor a previously unmonitored physiological parameter when the caregiver determines that a new physiological parameter should be monitored. The administrator selects through a dynamic licensing module the patient monitoring device and instructs the dynamic licensing system to enable monitoring of the new physiological parameter. The administrator defines how long the patient monitoring device is to be monitoring the new parameter. The administrator may also select locations in the hospital, and some or all patient monitoring devices at that location are upgraded to monitor the new parameter. The dynamic licensing system creates a licensing agreement to monitor the new parameter for the monitoring duration and/or in the monitoring location.

A variety of factors are input into the system with limited human intervention and a tool automatically determines the probability of whether the patient will have a better outcome if transferred or not. The factors include clinical factors, imaging factors and time to transfer factors. The tool includes processes for automatically determining several imaging factors, including the determination of clot length, collateral blood flow, the presence of forward blood flow within and around the clot, and the clot permeability. The tool has capability to continuously update the treatment protocol and other output results using current clinical, health system or other relevant information or feedback.
