  • Patent number: 11195133
Abstract: Systems and methods for individual risk factor identification include identifying common risk factors for one or more risk targets from population data. Individuals are stratified into clusters based upon the common risk factors. A discriminability of each of the common risk factors is determined, using a processor, for a target cluster using individual data of the target cluster to provide re-ranked common risk factors as individual risk factors for the target cluster, such that the discriminability is a measure of how a risk factor discriminates its cluster from other clusters.

Abstract: Methods and systems for managing care team assignment's for individuals, such as a patient, are provided. Embodiments include receiving indicators of actions that are initiated by a clinician and associated with a patient's care. Using times associated with each action, a time series indicating contribution levels for each clinician may be constructed. Care contribution curves measuring a clinician's care contribution level over time may be generated using the time series and a rate of decay for each action, which may be based on the type of action and the role of the clinician who initiated the action. A care contribution score for each clinician may be determined from the clinician's care contribution curve, and a care team assignment for the patient may be created based on care contribution scores for each clinician who initiated an action.

  • Patent number: 11114204
Abstract: A computer-based system to determine whether patients should be treated as inpatients or outpatients. The invention makes personalized predictions about the risk and timing of adverse outcomes for the patient, and further assesses how this risk and timing may vary if the patients are treated as inpatients or outpatients. This information informs how patients are assigned to an appropriate therapy. The invention includes logic relevant to predicting patient risk, decoupling patient risk into components inherent to the patient as well as additions/subtractions associated with the choice of treatment, and predicting the timing of adverse outcomes given censored data. The invention can be extended to use in a broad range of other application domains (e.g., matching learners to courses either offered in-classroom or online for education).

  • Patent number: 11114197
Abstract: A system associated with determining physician attribution related to in-patient care based at least on prediction of attribution values associated with patient-physician attribution is disclosed. The system extracts clinical data associated with a patient from a first database, with the extracted clinical data comprising clinical information related to the patient and the clinical information including vector values indicative of clinical progress of the patient at various stages of treatment. The system stores the extracted clinical data in a second database and determines attribution values using the attribution manager with the attribution values being based on a predictive analysis using predetermined weights associated with the vector values. A training data set is generated based on learned weight values associated with the clinical progress of the patient. The system iteratively updates the vector values of the patient at least using a predictive analysis associated with the training data set.

  • Patent number: 11081216
Abstract: Mechanisms are provided to implement a patient summary generation engine with deduplication of instances of medical concepts. The patient summary generation engine parses a patient electronic medical record (EMR) to extract a plurality of instances of a medical concept, at least two of which utilize different representations of the medical concept. The patient summary generation engine performs a similarity analysis between each of the instances of a medical concept to thereby calculate, for a plurality of combinations of instances of the medical concept, a similarity metric value. The patient summary generation engine clusters the instances of the medical concept based on the calculated similarity metric values for each combination of instances in the plurality of combinations of instances of the medical concept to thereby generate one or more clusters, and select a representative instance of the medical concept from each cluster in the one or more clusters.

  • Patent number: 11062238
Abstract: A method, system and medium are provided for enabling re-creation of a time-ordered sequence of clinical data. Sets of data elements for a patient are received in sequence from a clinical system, and for each data element a time stamp is attached. For any given set of data elements, a maximum time stamp is determined, implying that all events in a clinical system resulting in modifications to data elements contained in the set occurred on or before the maximum time stamp. A subsequent retrieval from a clinical system yields a set of data elements in which the time stamp for each data element occurs after the maximum time stamp from the previous set.
