Abstract: Devices and methods for delivering end effectors through surgical trocars are described herein. In one embodiment, a surgical end effector loading device is provided that includes a housing having a distal-facing first portion configured to couple with a proximal end of a surgical trocar, and an elongate second portion protruding distally from the first portion. The second portion can include a lumen formed therein and can be configured to extend into a working channel of the surgical trocar when the housing is coupled thereto. The device can also include at least one mating element coupled to the housing and configured to interface with a complementary mating element of the surgical trocar to restrict movement of the housing relative to the trocar, as well as an end effector retainer coupled to the housing and configured to selectively couple a surgical end effector to the housing.

  • Patent number: 10849630
Abstract: A reposable surgical clip applier includes a handle assembly, a shaft assembly releasably engagable with the handle assembly, and a clip cartridge assembly releasably engagable within the shaft assembly. The handle assembly includes at least one handle, a proximal drive member, a proximal pusher, and a handle spring assembly configured to bias the handle(s) towards a spaced-apart position. When the reposable surgical clip applier is assembled, the proximal drive member is positioned proximally adjacent an inner drive assembly of the shaft assembly such that movement of the handle(s) towards an approximated position actuates a jaw assembly of the shaft assembly. When the reposable surgical clip applier is assembled, the proximal pusher is positioned proximally adjacent a distal pusher of the clip cartridge assembly such that movement of the handle(s) towards the spaced-apart position loads a distal-most surgical clip from the clip cartridge assembly into the jaw assembly.

  • Patent number: 10835257
Abstract: Systems, methods, and devices for the treatment of acute ischemic stroke that provide immediate blood flow restoration to a vessel occluded by a clot and, after reestablishing blood flow, address the clot itself Immediate blood flow restoration advantageously can facilitate natural lysis of the clot and also can reduce or obviate the concern for distal embolization due to fragmentation of the clot. Several embodiments of the invention provide for progressive, or modular, treatment based upon the nature of the clot. For example, the progressive treatment can include immediate restoration of blood flow, in-situ clot management, and/or clot removal depending on the particular circumstances of the treatment. The in-situ clot management can include, for example, lysis, maceration, and/or removal.

  • Patent number: 10813634
Abstract: First and second arms have first and second retaining members, respectively. A switching mechanism switches a retained state of a first position of a tying medium between first and second states. In the first and second states, the first position is retained by the first and second retaining members at first and second retaining positions, respectively. A loop retaining member retains a loop around the second arm in a path extending from the first retaining position through second and third positions to a fourth position of the tying medium. The second position intersects the third position as a result of the loop such that the third position is closer to the first retaining position than the second position in the path. A moving portion moves the loop relative to the second arm to change a relative positional relationship between the loop and the first position on the path.

  • Patent number: 10813663
Abstract: A device suitable for use in a body lumen and comprises an elongated control member having a distal and a proximal end, and a radially expansible member disposed at or near the distal end and adapted for collection and/or shearing of matter (i.e. thrombus) from a wall of the body lumen (i.e. a vein or an artery). The radially expansible member has a proximal end and a distal end and is adjustable between a contracted orientation and an expanded orientation. The elongated control member comprising a proximal arm connected at or adjacent to the proximal end of the radially expansible member and a distal arm connected at or adjacent to the distal end of the radially expansible member such that movement of one arm relative to the other arm effects adjustment of the diameter or radial strength of the radially expansible member. The device comprises a control mechanism operatively connected to both arms and adapted to provide resistance to the movement of one arm relative to the other.

  • Patent number: 10799242
Abstract: Systems and methods are provided for repairing a heart valve, such as a mitral, tricuspid or aortic valve, using an adjustable and removable implant that can be delivered to the heart through the apex in a simplified and non-invasive manner. The implant can include a prosthetic valve portion coupled to a proximal end of a shaft, and an anchor portion coupled to a distal end of the shaft. The prosthetic valve can be suspended within an opening of the heart valve while the anchor portion is affixed to the apex of the heart. When the implant is deployed, a distance between the prosthetic valve portion and the anchor portion can be adjusted, and/or the implant or a portion thereof can be rotated to thereby change the position of the prosthetic valve within the heart valve. This can allow correcting for post-implantation movements of the implant to mitigate potential complications.

  • Patent number: 10646365
Abstract: A method for implanting a prosthesis centrally within a curved lumen includes loading a prosthesis into a delivery sheath, advancing the sheath in a patient towards the curved lumen to place at least the proximal end of the prosthesis within the curved lumen, and centering the proximal end of the prosthesis and/or the distal end of the sheath within the curved lumen. In a first advancing step, the outer catheter containing the inner sheath is advanced together towards the curved lumen to a location proximal of the curved lumen and, in a second advancing step, the inner sheath containing the prosthesis is advanced into the curved lumen to place at least the proximal end within the curved lumen while the outer catheter substantially remains at the location. After centering, the proximal end of the prosthesis is deployed centered within the curved lumen.

  • Patent number: 10624731
Abstract: A vascular filter system comprises a vascular filter (1), a clamp device (50), and a delivery catheter (51). The filter (1) comprises filter elements (6). When used without the clamp device (50), the filter elements (6) move from a closed state to an open state upon elapse of a predetermined period of time. In the closed state the filter elements (6) capture thrombus passing through the inferior vena cava (2). The delivery catheter (51) is employed to deliver the clamp device (50) to the filter (1) after the filter (1) has been deployed in the inferior vena cava (2). The clamp device (50) engages with the filter elements (6) of the filter (1) to clamp the filter (1) in the closed state beyond elapse of the predetermined period of time. Because of the presence of the clamp device (50), the filter elements (6) are no longer free to move from the closed state to the open state upon elapse of the predetermined period of time.
