  • Patent number: 10121558
Abstract: In a patient care process a human patient 2 is registered with a computer system 1. The system 1 records identity details for the patient 2 against a unique identifier. The system 1 receives information indicative of the health or other condition of the patient 2 and records such information against the patient's unique identifier. A health care plan 5 is also generated and/or recorded in or by the system 1 for the patient 2 against the unique identifier. The system 1 may receive and processes electronic data 6, 7, to determine whether the patient is substantially compliant or responding adequately to the care plan 5. The system 1 then generates and at least initiates a notification 8 to the patient 2 and/or at least one suitably authorized health care provider 3.

  • Patent number: 9830673
Abstract: A computer-implemented method for managing accounts in a diabetes management system. The method includes receiving, by an account manager, a request to deactivate a first account associated with a healthcare provider. The first account may be associated with a plurality of patient accounts. The account manager may be implemented as computer executable instructions executing on a computer processor of a server device. The method includes removing, by the account manager, access to data associated with the first account. Each of the patient accounts associated with the first account are identified and may be deactivated by the account manager. An electronic notification may be sent to a person associated with each of the identified patient accounts, advising the person that the corresponding patient account has been deactivated. An audit record may be created in a data store, the audit record indicating the deactivation of the first account.

  • Patent number: 9750862
Abstract: Methods for monitoring patient parameters and blood fluid removal system parameters include identifying those system parameters that result in improved patient parameters or in worsened patient parameters. By comparing the patient's past responses to system parameters or changes in system parameters, a blood fluid removal system may be able to avoid future use of parameters that may harm the patient and may be able to learn which parameters are likely to be most effective in treating the patient in a blood fluid removal session.

  • Patent number: 9734291
Abstract: CNA-guided care, which can improve clinical outcomes at a specific patient level and decrease total cost of care at the population level, has two faces. The first face, the enabling tool, allows a payer to transmit information identifying a health care service under consideration for a patient with a health condition whose health plan benefits cover the service and other payer-selected variables. In response, the payer receives information sufficient to establish that the medical service is an appropriate delivery or level of service allowing the payer to approve payment to a medical care provider for the service. The second face provides for communications that provide information sufficient for a complete medical evaluation and for assigning a CNA to the patient. The patient then can use the assigned CNA to select an optimal care plan and a medical professional based on patient-set criteria within the CNA.
