    Abstract: A mechanical device and method for gathering and securing tissue with a fastener. The device includes an applicator assembly configured to deploy a fastener in a first direction, a tissue manipulation assembly configured to move from a relaxed position to a grasping position in a second direction transverse to the first direction, and a translating trigger assembly coupled to the applicator assembly and the tissue manipulation assembly. The trigger assembly is configured to move from a relaxed position, through a first position, to a second position along a third direction transverse to the first and second direction. Operation of the trigger assembly from the relaxed position through the first position along the third direction causes the tissue manipulation assembly to move in the second direction to gather a portion of the tissue. Continued operation of the trigger assembly along the third direction causes the applicator assembly to deploy the fastener.

    Abstract: Current methods of drug administration to the lungs are inefficient. ‘Endotracheal Tube with Aerosol Delivery Apparatus III’ is specifically designed for uniform intrapulmonary delivery of aerosolized medication in patients on mechanical ventilation. As opposed to the current methods of drug delivery where aerosol particles are generated at the proximal end of the ETT, with majority of the particles adhering to the endotracheal tube during delivery, this invention bypasses the endotracheal tube by generating aerosol particles at its distal end. The invention consists of two coaxial hollow tubes fused to each other. The inner coaxial tube and/or one or more secondary cannulation(s) in the wall of the outer coaxial tube terminate at proximal and as MDI adapters and at the distal tip as a single or multiple micrometric orifices.

One or more devices can be implanted adjacent to or in contact with the stomach to occupy a space to prevent the stomach from expanding into that space as food is taken into the stomach. Alternatively, one or more devices may be implanted and expanded to displace at least a portion of the wall of the stomach to decrease the internal volume of the stomach that is available to receive food. Devices may be anchored to one or more internal structures at one or more locations without piercing through the wall of the stomach. Devices can be implanted using minimally invasive methods, such as percutaneous or laparoscopic methods. Delivery instruments are also provided. An intra-gastric sizing device is provided to facilitate implantation of an extra-gastric device in some method embodiments.

    Abstract: A fastener for insertion into pierced openings of a tissue wound has a body formed of a generally bioabsorbable polymer defining an initial capture area. The body includes a pair of arms, each with an inwardly projecting cleat operably joined at an elbow portion defining an internal elbow angle. The arms are operably joined to a backspan at a shoulder portion defining an internal shoulder angle. A durable tissue retention zone is defined between the cleat and the arm. The elbow portion and the internal elbow angle define an insertion width greater than a width of the pierced openings resulting in the pierced openings stretching over the cleat and being elastically retained within the durable tissue retention zone. The fastener captures wound tissue in the initial capture area and then dynamically reforms in response to lateral stresses without a fracture failure of the fastener until a minimum degradation period.

    Abstract: A medical fastener (18), preferably bioabsorbable, is used for repairing torn soft tissue, particularly meniscal tissue (16). The fastener (18) can have an enlarged head (22) at the proximate end to engage meniscal tissue (16) adjacent to a tear (14), and a pointed distal tip (21) at the other end to ease insertion of the fastener (18) into the meniscus (16). The shank (20) of the fastener (18) bridges across the tear (14). Opposite ends of the shank (20) are secured in the meniscal tissue (16) by a medical adhesive, preferably bioabsorbable, and preferably at locations remote from the tear (14) so as not to interfere with healing of the tear (14). The fastener (18) can have a blind bore (24) opening through the head (22) of the fastener (18) and extending through the shank (20) to a location close to the tip (21) and generally radial holes (26) communicating between the bore (24) and the exterior of the shank (20).
