    Abstract: A system for treating a target region in tissue beneath a tissue surface comprises a probe for deploying an electrode array within the tissue and a surface electrode for engaging the tissue surface above the treatment site. Preferably, surface electrode includes a plurality of tissue-penetrating elements which advance into the tissue, and the surface electrode is removably attachable to the probe. The tissue may be treated in a monopolar fashion where the electrode array and surface electrode are attached to a common pole on an electrode surgical power supply and powered simultaneously or successively, or in a bipolar fashion where the electrode array and surface electrode are attached to opposite poles of the power supply. The systems are particularly useful for treating tumors and other tissue treatment regions which lie near the surface.

The surgical drape extends from the inflatable thermal blanket and is sized to substantially cover the entirety of the patient's body.     Abstract: A sheath for use on a closed loop Joule-Thomson cryosurgical probe, and the combination of the and the closed loop probe. The sheath is slipped over the probe, thereby separating the probe from the environment. The sheath has a grip which fits over the handle of the cryosurgical probe, and an extendible shroud which can be longitudinally extended to cover tubing and which are attached to the handle. The sheath has a hollow multi-lumen catheter shaped and sized to fit snugly over the cannula of the cryosurgical probe. The catheter is not thermally conductive, preventing transfer of heat from the ambient to the gas mixture, and preventing the freezing of tissues at undesired locations along the catheter. A thermally conductive cap or tip is attached to the distal end of the hollow catheter. The thermally conductive cap or tip fits snugly over the cold tip on the probe, and it efficiently transfers heat from the target tissue to the cold tip, which in turn transfers heat to the expanded gas mixture.

In one aspect the invention provides a cutting tool or instrument with at least one cutting or coagulation tip and the tool is adapted to operate as both a power supply electrode to supply power to the tip and as a return electrode to allow impedance feedback signals to be received by a control means thereby eliminating the need for a separate second electrode and taking the patient out of the circuit received by a control means According to a second aspect the feedback information is stored, processed and analyzed in relation to pregrogrammed and previous actual feedback to allow alteration to the power supply to the tip to create optimum cutting and/or coagulating conditions.

    Abstract: A catheter includes a first plurality of expandable leads and a second plurality of expandable leads separate and longitudinally spaced-apart from the first plurality to deliver energy to a hollow anatomical structure, such as vein, fallopian tube, hemorrhoid, esophageal varix, to effectively ligate that structure. Each of the leads includes an electrode located at the distal end of the respective electrode lead. Polarizations of the leads may be selected to achieve the power distribution desired. Each electrode lead includes an outward bend such that when a movable sheath is moved out of contact with the leads, they expand outwardly into apposition with an inner wall of the structure to be ligated. High frequency energy can be applied from the leads to create a heating effect in the surrounding tissue of the anatomical structure.

    Abstract: An applanation tonometer with a replaceable prism for contacting a cornea has emitter and detector ports arranged opposite each other on opposite sides of a longitudinal axis, with the ports aimed at about 45 ° to the longitudinal axis. Light from an emitter then passes through the emitter port and crosses the prism axis to be incident and reflected from an emitter-reflecting surface of the prism and become incident on the applanation surface. Light reflected from the applanation surface is incident on and reflected from a detector-reflecting surface opposite and parallel with the emitter-reflecting surface to proceed across the prism axis to the detector port. Such an arrangement produces reliable applanation signals and varies the spacial intensity of light reflected from the applanation surface so that the tonometer can automatically distinguish between concentric and eccentric contact of the applanation surface with a cornea.

    Abstract: A clamping force mechanism and its method of use with electrosurgery allow a user to seal and/or join patient's particular vascular tissue; the mechanism is elongate with user and patient ends. An actuator is at the user end and the effectors are at the patient end. Each effector has a face of an area to contact the particular vascular tissue. A lost motion connection transfers user actuation to the effectors to hold a predetermined clamping force during electrosurgical tissue sealing. A yielding member in the loss motion connection clamps the particular tissue between the faces with a predetermined force. The yielding member is a spring, slip clutch or hydraulic coupling possibly near the actuator. An active electrode is carried on one end effector and a return electrode contacts the tissue so an electrosurgical energy supply connected thereacross delivers energy therebetween. A feedback circuit responds to parameters of energy delivered to tissue.

A wide bandwidth double-disk antenna (700) is effective for this purpose over a bandwidth of about six gigahertz. A computer simulation provides initial screening capabilities for an antenna such as antenna, frequency, power level, and power application duration. The simulation also allows optimization of techniques for specific patients or conditions. In operation, microwave energy between about 1 Gigahertz and 12 Gigahertz is applied to monopole microwave radiator (600) having a surface wave limiter (606).

    Abstract: A catheter includes a plurality of primary leads to deliver energy for ligating a hollow anatomical structure. Each of the primary leads includes an electrode located at the working end of the catheter. Separation is maintained between the primary leads such that each primary lead can individually receive power of selected polarity. The primary leads are constructed to expand outwardly to place the electrodes into apposition with a hollow anatomical structure. High frequency energy can be applied from the leads to create a heating effect in the surrounding tissue of the anatomical structure. The diameter of the hollow anatomical structure is reduced by the heating effect, and the electrodes of the primary leads are moved closer to one another. Where the hollow anatomical structure is a vein, energy is applied until the diameter of the vein is reduced to the point where the vein is occluded. In one embodiment, a balloon is inflated to occlude the structure before the application of energy.

    Abstract: A method and apparatus for protected ablation of anatomical tissue. A protective cap is disposed around tissue within an anatomical cavity by an endoscopic device or through open surgery techniques. The tissue can then be subject to ablation at high energy for a relatively long period of time, if necessary to ablate the tissue, without damaging surrounding organs or other tissue. The protective cap can be inserted endoscopically in a closed position in which it is compact radially and can be opened after being inserted into the body cavity to be placed around the tissue. The protective cap can be a thermal insulator, an electrical insulator, merely a physical pad to protect surrounding tissue, or of any desired characteristic for protection.

    Abstract: A surgical handpiece tip with at least two coaxially spaced electrically conductive tubes. The tubes are separated by an electrical insulator. The interior of the inner tube is used for aspiration of liquefied tissue. The distal portion of the interior tube terminates just inside of the outer tube so as to form a boiling region. Electrical current is passed between the inner and outer tube to rapidly boil any surgical fluid in the boiling region. The boiling fluid rapidly expands out of the ring between the tubes and contacts the targeted tissue, thereby liquefying the tissue and allowing the tissue to be aspirated.
