    Abstract: A patient uses a token with multiple actuators to request assistance from a healthcare provider. When the patient activates one of the token's actuators, a nearby mobile device receives a signal from the relevant actuator, selects a code depending on which actuator was activated, and sends the code along with location data and a patient identifier to a server. The server uses the identifier to access a patient record for the patient and retrieve insurance data. The server will use the code, the location data and the insurance data to select an appropriate healthcare provider for the patient. The system will transmit a communication to the selected healthcare provider with a phone number for the patient's mobile device so that the healthcare provider can contact the patient.
