The processor(s) cognitively analyzes the data to identify baseline behavioral patterns of the individual, when the individual is engaged in each of the physical activities. The processor(s) obtains data indicating consumption of a substance by the individual at a first time. The processor(s) determines impacts of the consumption on the baseline behavioral patterns of the individual and generates a data structure (a predictive model) that includes expected deviations from the baseline behavioral patterns of the individual, when the individual has consumed the substance.

Abstract: A system for evaluating health-related quality of life (HRQOL) can include a sensor adapted to generate sensor data for a user, a memory adapted to store the sensor data, and a processor coupled to the sensor and the memory. The processor can be configured to initiate executable operations including determining a first biological marker for the user from the sensor data, wherein the first biological marker is indicative of a first dimension of HRQOL, performing speech analysis on user provided speech to determine a second biological marker indicative of a second dimension of HRQOL. The processor is further configured to initiate executable operations including comparing the first biological marker and the second biological marker with a baseline for the first biological marker and a baseline for the second biological marker, respectively, and outputting a result of the comparing.

Abstract: A method for ensuring hand hygiene compliance may include tracking dispensing of a plurality hand hygiene dispensers having hand sanitizer. Each hand hygiene dispenser can be associated with an individual user. A processor may determine dispensing rates of the plurality of hand hygiene dispensers for a plurality of predetermined time periods and each of the plurality of the hand hygiene dispensers is identified with an individual user. The average dispensing rate can be calculated taking a predetermined number of highest dispensing rates out of the plurality of predetermined time periods. The rates can be displayed to the users, and the users can be notified of proper hand hygiene compliance or if the average dispensing rate falls below a target threshold rate.

Abstract: When calculating an individual's health index as a function of health index parameters including biometric parameters and behavioral parameters, a monetary cost is associated with each health index parameter, and the costs for all health index parameters are aggregated and displayed to a user via a graphical user interface. The user is then permitted to adjust one or more health index parameters, and the aggregate health cost is updated to show the user the amount of health cost savings that can be achieved by different health index parameter adjustments. In this manner, an individual is incentivized to improve his health in order to save money.

Abstract: A method for generating a treatment plan in response to medical test results is provided. The method comprises requesting point-of-care (POC) services from a mobile POC unit, receiving at a server confirmation of delivery of POC services to a patient, receiving at the server one or more test results as a result of operation of a medical testing device used in the POC services, wherein the one or more test results includes a determination of the efficacy and dosage level of a medication, generating at the server an updated digital patient record reflecting the one or more test results, and transmitting by the server to a medical entity a treatment plan based on the efficacy and dosage level determined for the medication, wherein the treatment plan is a dosage regimen for the medication.

The physician profile database stores physician profiles, each of which contain information associated with a physician. The system further includes a physician referral application which is hosted at least partially on a server and electronically accessible over at least one network system to a primary care physician device and a specialist physician device. The physician referral application is configured to facilitate a referral of a patient between the primary care physician device and the specialist physician device, and to allow a user of at least one of the primary care physician device and the specialist physician device to search for physicians based on information contained in the physician profiles. Abstract: A computerized system and method for automatically estimating the likelihood of having a fall leading to a fracture/dislocation within a specified period is described, and comprises a predictive model for guiding patients to the right course of treatment and encouraging discussions with their doctors for better outcomes. The system and method extracts member's health information from health administrative claims data, including clinical and pharmacy data, and estimates the probability of a fall for that member. Patients with high risk scores are selected for various clinical programs and interventions to manage their health conditions and reduce their likelihood of falling.

Abstract: A prescription management system receives prescription information and manages containers filled with the prescription. The prescription is stored in the container and the container is attached to a tracking device storing prescription information. The tracking device is managed by the prescription management system. The tracking device activates an indicator when it receives a request from the prescription management system identifying the tracking device. The indicator permits a user, such as a pharmacist, to locate a desired prescription. In some instances, filled prescriptions may be stored in a filled prescription holding area for a long period of time (i.e., the customer hasn't picked up the prescription). The system allows a user to identify these filled prescriptions and return the medication back to available stock for filling future prescriptions. The system may also identify these filled prescriptions and have the filled prescriptions delivered to a customer's home before the prescriptions expire.

Abstract: Aspects of the present disclosure describe systems and methods that dynamically de-identifies confidential and/or sensitive patient health data maintained at computing devices deployed within a communications network. A key value is securely transmitted from a remote device included in a first security domain to a first server included in a second security domain. Based on the key value, the first server accesses a dataset containing health records identifying individual patients and de-identifies all or part of the dataset. A second server included in a security domain different from the remote device and the first server matches the de-identified data to previously de-identified data and generates a unique alias identifier for the matched de-identified data.
