Abstract: A system and method for providing a centralized source of health care information for an individual. Security measures for access to health care information of an individual may be provided. An interactive patient interface may be employed by the individual to gain access to the health care information, and an interactive provider interface may be employed by one or more health care providers of the individual to gain access to the health care information. Access and changes to the health care information from any of the health care providers and individual may be allowed as a function of a security level granted thereto.
