    Abstract: Embodiments of a device and method for connecting a bone fastener, for example a spinal screw, to an elongated member are disclosed. In one embodiment, a body having channels to receive the fastener and elongated member is tapered and has an aperture for a locking member. A tapered washer slides over the body. The tapered washer has an inner diameter that is smaller than part of the tapered portion of the body allowing it to engage the body. The fastener passes through the body and engages the washer. An offsetting member slides over the body. The body slides over the elongated member and the offsetting member offsets the elongated member from the spinal fastener and contacts each in two differing locations. A locking member is inserted into a threaded aperture in the body to apply a compressive force that compresses all of the members preventing relative motion of the components.

    Abstract: A multi-axial bone fixation implant includes an elongated member, one or more bone anchor assemblies, and stabilizer members fitted within the elongated member. A bolt having a bone-threaded end and a machine-threaded end and an enlarged medial portion is also provided. The bolt is anchored into a bone, and its machine-threaded end passes through the elongated member and the stabilizer, and is engaged by a coupled washer and nut. The washer includes an oblong aperture, and the nut includes a projecting sleeve. The sleeve is inserted into the washer and expanded, and the washer retains the sleeve without impairing relative rotatability or translatability of the nut and washer. When locked by the nut and washer, the enlarged portion of the bolt is forced against the stabilizer, which locks against the elongated member. The elongated member can be fixed with respect to the anchor at an infinite number of multi-axial angles.

The system includes an elongated member, one or more bone anchor assemblies, and stabilizer members which are fitted within the elongated member. A bone anchor is attached to a bone, and the elongated member and stabilizer are fitted over the bone anchor. A rounded washer and nut having a corresponding rounded underside surface are fitted on to the bone anchor over the elongated member, and tightened. The configuration of the bone anchor assembly, including an intermediate portion of the bone anchor, the arcuate washer, and the nut, along with the configuration of the sliding support, allows multi-axial positioning of the bone anchor with respect to the elongated member at a plurality of locations along a slotted member.

When the closure member is threaded into the receiver member, forces tending to splaying the wall portions or legs of the receiver member cause an interference fit between thread surfaces of the closure member and the receiver member, preventing the splay from occurring.
