Abstract: A method of facilitating health of a patient includes determining, based on patient information, a risk score. The patient information includes clinical information and at least one of psychosocial information, experiential information, relational information, preferential information, demographic information, barrier information, and compliance information. The method further includes identifying a number of risk factors that influence the risk score; determining a level of influence that each risk factor has on the risk score; and causing a display device to present a representation of the risk score.
