The device comprises a mountable housing for mounting under a shelf, or a wall or the like in an unobtrusive or discreet manner. The housing has a first flat configuration for transportation and storage and a second three-dimensional configuration for device deployment. The device housing further includes an insert having an adhesive surface and a dispenser comprising pheromone, kairomone or other attractant, lure or bait, or, alternatively, a repellant or a combination of any of these. The adhesive is effective for trapping multiple orders of insects. A plurality of attractants, etc., is simultaneously used for attracting multiple orders of insects. The insert is selectable, removable and replaceable.
