    Abstract: A patient wristband form has a transparent ply with an upper surface and a lower surface, and a pressure sensitive adhesive coating on the lower surface. A release ply has an upper surface and a lower surface, with a release coating on the upper surface. The transparent ply is removably mounted on the upper surface of the release ply by the pressure sensitive adhesive coating. A die cut in the transparent ply defines an elongated wristband. An opaque coating is provided on the upper surface of the transparent ply in a central portion of the elongated wristband. A die cut in the transparent ply defines an overlaminate label that is sized to cover at least a part of the central portion of the elongated wristband, thereby covering indicia printed on the opaque coating. A paper ply may also be mounted on the release ply by pressure sensitive adhesive. The paper ply defines a plurality of labels that may be printed at the same time as the wristband.

    Abstract: A self-adhesive label is provided for displaying useful information about an object to which the label is affixed. The label is formed with a face layer constructed out of heavy paper, such as cardstock, and having a first planar surface which is suitable to receive printed indicia thereon. An adhesive layer coats a second planar surface of the face layer and includes a pocket-forming window therein, which is in communication with a slot in the face layer, and adhesively affixes the face layer to the object. The pocket-forming window defines an item receiving pocket between the object and the face layer, wherein the item-receiving pocket is accessible through the slot, for example, to removably insert a card therein. A sheet of labels is also provided having two or more labels thereon, wherein each of the two or more labels are removable from the sheet for individual use thereof.

The face sheet defines first and second major faces and includes printed indicia on the first major face. The printed indicia being arranged to indicate an attempted delivery. The reusable adhesive laminate includes a reusable adhesive and a removable ply. The reusable adhesive and the removable ply are configured such that the reusable adhesive retains its properties of adhesion following removal of the removable ply therefrom. The laminating adhesive is interposed between the liner ply and the face sheet so as to secure the liner ply to the second major face of the face sheet. The laminating adhesive is further interposed between the reusable adhesive laminate and the face sheet so as to secure the reusable adhesive laminate to the second major face of the face sheet.

    Abstract: A construction for a laminated window label is provided and includes a transparent film having an adhesive on one surface thereof, liner stock having first and second release-coated surfaces which is adhered on one surface to the transparent film, and an opaque layer which is adhered to the other surface of the liner stock. The transparent film and opaque layer preferably include die cut label portions. Variable and nonvariable information may be printed on the die cut label portion of the transparent film, and the opaque layer may be removed from the construction and replaced onto the surface of the film, thereby laminating the label portion of the opaque layer to the label portion of the transparent film. The two label portions may then be peeled away from the remainder of the construction as a single label which is then applied to a transparent surface such as a window.
