The memory holds instructions that when executed by the processor cause an apparatus to perform displaying on a display of a client device an identifier of a patient in a list of patients from a patient database, outputting a first input object on the client device for user input indicating health conditions of the patient, outputting a second input object on the client device for user input confirming at least one of an admission status of the patient or a discharge status of the patient, and providing to the patient database an identifier of the selected patient, the user input indicating the health conditions and the admission status or the discharge status of the patient.

    Abstract: Techniques are described herein that assist a medical provider in submitting a referral for a patient (e.g., member). A service provider may identify one or more locations and/or one or more providers capable of conducting a procedure associated with the referral. The location(s) and/or provider(s) may be determined based on respective locations associated therewith, a patient location, a level of quality associated therewith, a cost associated with the procedure at the respective location(s) and/or provider(s), and the like. In various examples, the service provider may receive the referral from a first medical provider and may determine a risk associated therewith. The risk may be determined based on data associated with the patient, the first medical provider and/or a second medical provider associated with the referral. In various examples, the service provider may determine to automatically approve the referral based on the risk associated therewith.

    Abstract: Methods and systems establishing a telehealth session between a patient and a healthcare provider (HP) are disclosed herein. One of the systems includes: a HP-side application residing a HP-side device; a patient-side application residing on a patient's device; and a telehealth application residing on a server. During a telehealth session, the HP-side application is configured to: generate one or more graphical user interfaces (GUIs) that display patient-related data on the display of the HP-side device; enable the healthcare provider to edit, using the one or more GUIs, one or more portions of the patient-related data; and send the one or more edited portions of the patient-related data to the telehealth application on the server for storage.
