    Abstract: A method and apparatus for remotely providing survey assessment instruments to users includes selecting one or more instruments, a user identification and a personal identification number and associated those components into an experience. The experience is stored in a database that is remotely accessed by a user who has been provided with the user identification and the personal identification number. Upon confirmation of the association, the user is permitted to access the experience which access can be controlled by associated rule and threshold settings.
