The PHR server maintains the PHR of an owner, the PHR including a plurality of sections, one of the sections being an aggregated section including a plurality of subsections, one of the subsections corresponding to a health care entity. The method includes receiving a retrieval input from the owner of the PHR, the retrieval input identifying the health care entity from which information is to be requested. The method includes generating a request for the information. The method includes transmitting the request to the health care entity. The method includes receiving the information from the health care entity. The method includes including the information in the subsection of the aggregated section in the PHR corresponding to the health care entity.
