Abstract: A healthcare self-management intervention system and method monitors, visualizes and rewards users for participating in activities related to the self-management of a medical condition, considering channels such as diet, exercise, medication, testing, scheduling/coaching, and shopping. It is a delivery system for treatment protocols which can monitor patient engagement with said protocols through a telehealth intervention system. A digital portal may be provided for analyzing and visualizing biometric data, presenting treatment protocols and educational materials, conducting remote teleconference meetings, sending data to and from remote locations in real-time to facilitate remote appointments and grade user progress towards or away from achievement of wellness goals.
