    Abstract: A business form with an integrated label and/or fold-over card is provided. The form includes a form layer having a top surface, a bottom surface, and a periphery, at least one die cut though the top and bottom surfaces within the periphery of the form layer defining at least one portion of the form layer within the die cut, and an adhesively but removably secured patch layer. The patch layer may be secured over the entire die cut and form layer portion to form an integrated label intermediate, or half of the patch layer may be secured over the die cut and form layer portion to form a fold-over card intermediate. In both embodiments the adhesive layer remains on the bottom surface of the patch when the label or card intermediate is removed from the form. By avoiding the use of any release materials, liners, layers and coatings, the remaining form layer is completely recyclable.

    Abstract: A business form with laser compatible punch-out card and the methods of making the combination. The card is permanently adhered to a perforated backer sheet, the periphery of which extends beyond the card and is coated with adhesive for application to the bottom surface of a form containing a hole such that the card is disposed within the hole. The card and attached portion of the backer sheet can then be removed from the card-in-form combination by breaking the perforations. The card is die cut from card stock laminated to the backer layer and is prepared for application to the form by stripping a waste matrix of card material from the laminate through the use of patterned release material.

    Abstract: A form with a well has a form layer with a top and a bottom surface and a perimeter, a die cut through the form layer that defines a portion of said form layer within said die cut, a patch layer having a perimeter and top and bottom surfaces, and an adhesive layer that is repositionable and peelable. The patch layer is adhesively but removably secured to the bottom surface of the form layer such that the die cut and form layer portion are entirely within the perimeter of the patch layer. The form has a well, that is created by removal of the form layer portion from within the die cut. The bottom of the well is the top surface of said adhesive layer and the sides of the well are the edges of said die cut. The adhesive layer has a greater affinity for the patch layer than the form layer portion such that when the form layer portion is removed from the form, the adhesive stays adhered to the patch layer. Articles such as photographs may then be inserted into the well.

The laminated card is created from a multiple-ply structure defined within the boundaries of the larger form. The multiple-ply structure has horizontal dimensions substantially corresponding to the ultimate horizontal dimensions of the laminated card or article being created. The carrier is printable and able to be laminated on both sides, yet if desired, the carrier can be limited to only a single ply, thereby reducing the thickness of the resulting laminated card.

    Abstract: A stencil apparatus which can be releasably secured to an upper surface of a substrate. The stencil apparatus comprises a stencil material which is deformable to accept impression of indicia and also enables the reproduction of the impressed indicia on another surface. The stencil apparatus further can include a substantially continuous frame which defines an opening and is attached to a marginal portion of the stencil material. The opening is large enough to permit the stencil material to contact the substrate in an unobstructed manner for transfer of the impressed indicia onto the substrate. The present invention is cleanly and releasably secured to the substrate. The apparatus attaches such that objectionable residues do not remain on the substrate and the stencil apparatus after the stencil apparatus is released therefrom. The apparatus is designed to be producible in a roll which is suitable for use on label affixing equipment.
