For spinal stabilization, the plate is attached at least to first and second vertebrae with at least one bone fastener in each vertebra. Retaining members are attachable to the plate to prevent the bone fasteners from backing out of the plate. One retaining member is a variable retaining member that allows the bone fasteners to toggle in the plate without interference. A second retaining member is a fixed retaining member that fixes the bone fasteners relative to the plate.

    Abstract: An intervertebral disc prosthesis in a preferred embodiment has a matrix of bioincorporable fabric, and a nuclear core centrally mixed into the matrix. The core is formed by impregnating the fabric substrate centrally with a polymer, preferably of liquid form that cures into a viscoelastic solid, in which each component—polymer and fabric—reinforces the other against tearing, shearing and weakening under stress. The core is a hybrid composite adapted for elastic deformation centrally of the matrix, in which the polymer is mixed with the fabric, and is surrounded by the outer bioincorporable fabric margin of the matrix. In another embodiment, the nuclear core is separated from an outer sheath of the bioincorporable fabric by an intermediate ligament encasement that surrounds the purely polymeric core In either embodiment, each edge of the outer fabric that interfaces a vertebral end plate is impregnated with an agent to stimulate osseus incorporation and anchoring.

    Abstract: A plating system for stabilizing at least one level of the spinal column includes a plate member having a first portion with a bottom surface extending between opposite sides of the plate member. The bottom surface is positionable extradiscally along first and second vertebrae for engagement of the plate member thereto. A second portion of the plate member extends from one side of the first portion and away from the second side of the first portion. The second portion is received in the disc space between the first and second vertebrae and intradiscally engageable to at least one of the first and second vertebra.

The upper node (8a) has a hole (8) through the plate (7) to receive a screw (11) for passage into the L5 vertebra. The lower nodes (9a, 10a) each include a hole (9,10) through the plate (7) to receive screws (12, 13) for passage into the S1 vertebra. The system further includes a plate (7) having a retaining element (32) for preventing backout of screws inserted through the plate (9). Instruments and methods for attaching the plate to the spinal column are also provided.
