    Abstract: A method of positioning a device for pedicle access includes securing a clip in an adjustable frame assembly and securing the device in the clip. The method also includes positioning the device at an initial position having a trajectory estimated to allow the device to penetrate a pedicle without damaging the nervous system, and determining whether the trajectory of the initial position is proper to avoid damage. Further, the method includes re-positioning the device to a new position if the trajectory of the initial position is not proper to avoid damage to the nervous system, and determining whether the trajectory of the new position is proper to avoid damage. The method also includes repeating the steps of re-positioning the device and determining whether the trajectory of the new position is proper as required until a proper trajectory is obtained.

In a first position the first and second refractor blades are laterally opposed. Movement of the securement feature adjusts the angle of at least one of the retractor blades so that at least one of the retractor blades may be adjusted between an acute angle and an obtuse angle to horizontal.

    Abstract: Surgical retractor systems including removable angling devices and methods of use are provided. A surgical retractor system can include a head with an opening, a blade including a nipple, and an angling device including a handle. The nipple can include a first recess that runs circumferentially around an exterior surface of the nipple and terminates at a flange. The opening can be configured to receive the nipple. The nipple can be longer than the opening such that the flange and at least a portion of the recess extend beyond an end of the opening when the blade is attached to the head. The angling device can be configured to receive the flange such that the angling device can be removably attached to the flange. The angling device can be used to manipulate the position of the blade when the angling device is attached to the flange.

    Abstract: A novel dilation system for providing access to a surgical site of interest is provided. The dilator includes a body, an arm, and a plurality of articulating dilation members pivotally connected to the body. The body includes a cavity, and the arm is configured to be accepted by the cavity of the body. The arm includes a distal portion and a proximal portion. The plurality of articulating dilation members are operably connected to the distal portion of the arm, and are movable between a closed position and an open position. The dilator may define a distally tapering generally conical volume in the closed position, and a generally cylindrical volume in the open position. The articulating dilation members are moved from the closed position toward the open position by the arm when the arm is articulated distally, and moved from the open position to the closed position by the arm when the arm is articulated proximally.

    Abstract: A spacer for implantation between adjacent vertebrae is provided. The spacer includes a distal end and a proximal end. The spacer also includes top and bottom surfaces spaced by sides. The top and bottom surface define a height, and the sides define a width. Each of the sides of the spacer may include a depressed region sunk into the side including a sloped surface at least toward the proximal end of the spacer. The distance between the sloped surfaces of the sides may decrease proximally to form a web having a leading edge proximate to the proximal end of the spacer. The web may be sized and configured to aid distribution of bone graft material to either side of the spacer, wherein bone graft material is supplied to a site of interest is distributed to at least one side of the spacer.

The top and bottom surfaces define a height, and the sides define a width. The height generally increases distally and from the second side to the first side. The top and bottom surfaces are curved, and include multiple radii of curvature. A lateral curvature is defined as the curvature along the top or bottom surface at a given point along the length of the spacer, taken along a plane extending between the first and second sides and generally perpendicular to the sides. The radius of the lateral curvature for at least one of the top and bottom sides varies along the length of the spacer. For example, the radius of curvature may generally increase distally along the length of the spacer.

    Abstract: Certain embodiments provide surgical retractors that include an arm and a blade, the arm including an opening and a first notched surface comprising a plurality of notches that extend radially outward from the opening, the blade including a nipple and a second notched surface comprising a plurality of notches that extend radially outward from the nipple. Attaching the blade to the arm such that the notched surfaces mate can secure the blade relative to the arm such that the blade cannot swivel radially about the opening. Certain embodiments provide methods for securing a retractor blade that can include inserting a nipple on a retractor blade into an opening and mating a first notched surface comprising a plurality of notches that extend radially outward from the opening with a second notched surface comprising a plurality of notches that extend radially outward from the nipple.

    Abstract: An adjustable surgical retractor clamping mechanism for attaching mechanism to an operating table. The mechanism includes an upper jaw, lower jaw, a drive member, an adjustable frame support post and a releasable clamp for adjusting the height of the surgical retracting frame. The upper portion of the lower jaw has a plurality of slots cut into the sidewalls of upper jaw to form at least one flexible finger. The clamp is in close sliding relationship to the upper portion of lower jaw member. The clamp includes a cam lever. Movement of the cam lever to its closed position compresses the clamp causing the flexible fingers to grip the adjustable post locking its longitudinal movement. Rotation of the cam lever to the open position releases the compressive force on the clamp, unlocking the adjustable post allowing the post to be longitudinally adjusted relative to the clamping mechanism.

    Abstract: A bone collection system includes a plunger, a tube, and a manifold. The plunger has a bore therethrough, an insertion portion, and an inlet that accepts material being harvested from a patient. The insertion portion includes a filter to prevent passage of bone fragments. The tube accepts the insertion portion, defining a first and a second chamber of the tube. The filter separates the first and second chambers of the tube. The manifold is accepted by the tube, and has an outlet accepted by a suction source. The inlet of the plunger is in fluid communication with the first chamber, the first and second chambers are in fluid communication through the filter, and the second chamber is in fluid communication with the outlet. Material being harvested is drawn through the bone collection system, and bone fragments of the desired size collected in the first chamber.

    Abstract: A surgical port and dilator are useful for creating access to surgical sites having tortuous geometries and/or requiring relatively long and narrow access to the surgical site. The surgical port or dilator can be provided with an oblique distal end to facilitate placement and the displace tissue near angled surfaces. The surgical port can be inserted into the operative site following dilation in the same fashion as a conventional port. Once inserted, however, the surgical port may then be elongated to provide relatively long and narrow, elliptically- shaped access to the surgical site. The elongation of the port allows the surgeon access to long and narrow surgical sites, such as the spine, without having to expand radially. This reduces trauma to the patient. A locking means is also provided to hold the surgical port in the open position until a release mechanism is activated.
