Abstract: The present invention provides systems, methods, and devices for assessing a patient for medical information, and preparing and sharing a report thereof via a networked environment, such as the Internet. In some embodiments, the questions asked of a patient are uniquely tailored to avoid irrelevant questions and to explore relevant information more deeply than one-size-fits-all medical forms. Further embodiments relate to the analysis of answers provided, and the assignment of risk scores, the calculation of risk stratifications, and the identification of triggers prompting action on behalf of the patient, to be included in the report.
