Abstract: Implementations of the present disclosure include methods, systems, and computer-readable storage mediums for tracking and communicating health condition of patients. In some implementations, a medical procedure associated with a patient is received. A plurality of patient records associated with the medical procedure are obtained from a database. The plurality of patient records includes medical histories of a plurality of patients. A questionnaire is generated by applying machine learning techniques on the plurality of patient records. The questionnaire is provided to the patient. A set of answers for the questionnaire is received. A health score is calculated for the patient based on the set of answers and the plurality of patient records. Based on the health score, a health notification associated with the patient is provided.
