Conduit (117) is arranged within a port (125) which includes a screw-threaded wall (126). The sleeve nut (124) includes a cylindrical body (146) having an inwardly facing cylindrical wall which is arranged to define a cylindrical air gap (148) between itself and an outer wall (147) of conduit (117). Region (150) of the sleeve nut is screw-threadedly engaged in wall (126) of the extruder. In use, cool compressed air is introduced into the assembly in the direction of arrow (170) and it flows through the assembly to cool it.

A patient may participate in a collection of surveys during the course of disease treatment that are automatically tailored to the patient's disease state using multidimensional tools to generate quality of life metrics. Reports are generated from the aggregate data to aid in treatment of the patient by enhancing patient/healthcare provider communications, patient education and by giving the healthcare provider reports on quality of life metrics correlated to the prescribed treatments, comorbid diseases, disease specific and medication specific review of systems and patient compliance with the prescribed treatments. Further, data from multiple patients are aggregated for reports that can provide evaluations of the effect of prescribed treatments, reasons for patient non-compliance with prescribed treatments and the prevalence and effect of off-label use of medications.

    Abstract: Method and apparatus for controlling access to or from a facility associated with providing services and/or sales of merchandise. The facility has an automatically operated, normally closed access gate and a scanner. Patrons may access the facility free of charge or at a discount by displaying an article of merchandise such that the scanner reads a bar code inscribed upon the merchandise. Data corresponding to the scanned bar code is compared to data entered into the memory of an automated data processing device. If that comparison results in establishing a correlation between data from the scanned merchandise and data entered into memory corresponding to merchandise for which a discount is accorded, then a control signal is generated and transmitted to the access gate. The access gate then opens and allows the patron submitting merchandise for scanning to enter or leave the facility.

Data corresponding to the scanned bar code is compared to data entered into the memory of an automated data processing device. If that comparison results in establishing a correlation between data from the scanned merchandise and data entered into memory corresponding to merchandise stocked by the sales facility for vending, then a control signal is generated and transmitted to the access gate. The access gate then opens and allows the vehicle submitting merchandise for scanning to access the parking facility.
