    Abstract: A surgical stapling or fastening instrument for applying surgical fasteners to tissue having an adjustable closure mechanism to linearly approximate the distance between the jaw members of the instrument. The adjustable closure mechanism consists of an advancing mechanism and a retaining mechanism, wherein the actuating mechanism is actuable to urge the jaw members towards each other. A coupling arrangement is also provided which permits firing of the staples or fasteners only when the jaw members are approximated a predetermined distance from each other.

    Abstract: A surgical instrument for anastomosis of first and second blood vessels is provided having a handle and a body portion extending distally from the handle. A fastener support is mounted adjacent a distal end portion of the body portion. A plurality of surgical fasteners are releasably supported by the fastener support at the distal end thereof. The surgical fasteners each have a leg with an atraumatic tip. A fastener camming member is mounted adjacent the fastener support. The fastener camming member and fastener support are relatively slidable in response to actuation of the handle to simultaneously deform the surgical fasteners to secure the first and second vessels without piercing the vessels.

    Abstract: A valve assembly adapted for introduction of an elongated object into a patient's body having a first valve formed of a resilient material and defining an aperture for reception of the object, the aperture being configured and dimensioned such that insertion of the object into the aperture will cause the resilient material defining the aperture to resiliently engage the outer surface of the object in a fluid tight manner. A second valve is positioned adjacent and distal of the first valve in general alignment therewith, whereby the second valve defines an aperture in general alignment with the aperture of the first valve, and is formed of a flexible material at least in the region defining the aperture. A pair of manually operable clamps are provided to selectively permit the aperture of the second valve to be opened or closed so as to permit entry of the object such that the object first passes through the first valve and then the second valve prior to entry into the patient's body.

    Abstract: An apparatus and method are disclosed for applying a surgical clip to an exterior wall of a blood vessel to at least partially close a hole formed therein. The apparatus includes a handle portion, an elongated body extending distally from the handle portion and dimensioned to extend through a hole in the wall of a blood vessel, and a collapsible locator associated with a distal end portion of the elongated body and mounted for movement between a collapsed position and an expanded deployed position. The locator is adapted to expand within an interior lumen of the blood vessel to maintain the distal end portion of the elongated body in a desired location with respect to the hole in blood vessel wall such that a surgical clip releasably supported adjacent the distal end portion of the elongated body can be applied to the exterior wall of the blood vessel to close the hole formed therein.

    Abstract: Apparatus is disclosed for applying surgical fasteners to body tissue along an arcuate path. Each surgical fastener may be in the form of a two-part fastener having a pronged fastener portion for piercing body tissue, and an apertured retainer dimensioned and configured for engaged reception of the pronged fastener portion in interference fit therewith for gripping the body tissue therebetween. Alternatively, the fasteners may be in the form of deformable staples. The apparatus preferably includes an arcuately shaped member for holding a plurality of the fastener portions in generally aligned relation, a correspondingly arcuately shaped member spaced from the fastener portion holding member for gripping body tissue therebetween and for releasably holding a plurality of the retainers in generally aligned relation and positioned opposite the fasteners when the body tissue is positioned therebetween.

The body of the apparatus comprises a frame housing an actuating mechanism and a transmission mechanism for transferring movement to the pusher bar and channel. The endoscopic portion is rotatable relative Go the frame. In one embodiment the apparatus is a single disposable unit. In an alternative embodiment the endoscopic portion is detachable and disposable whereas the non-endoscopic body portion is reusable.

    Abstract: An apparatus for inserting a staple into torn tissue such as the meniscus of the knee. The apparatus includes a pair of shafts individually movable in a longitudinal direction which are actuable from a handle mechanism. The shafts are movable sequentially so that the shafts disjunctively advance the prongs of the staple which is releasably held adjacent distal end portions of the shafts. A further embodiment of the device consists of a pair of needles detachably secured to a pair of anchoring members having a plurality of barb-like projections extending outwardly therefrom. The anchoring members are joined by a suture which connects adjacent the trailing ends of the anchoring members opposite the penetration end of the needles.

A collar which is provided in the elongated portion translates longitudinally to draw a cable which operates to move the anvil member relative to the cartridge assembly, thereby clamping body tissue to be fastened. A self contained pneumatic system is associated with the frame portion and is actuable to eject the surgical fasteners from the cartridge assembly.

    Abstract: A handle for a laparoscopic instrument is provided for manipulating a distal laparoscopic tool. The handle includes a casing, a pivot handle pivotally attached to the casing, and an actuating shaft mechanism extending outward from the casing. Attached at the distal end of the actuating shaft opposite the casing is the laparoscopic surgical tool. The actuating shaft mechanism is longitudinally movable in a first direction by a pivotal movement of the pivot handle for manipulating the laparoscopic tool. A detent carried by the casing allows movement of the shaft longitudinally in the first direction while restraining longitudinal movement of the shaft in a second opposite direction. A thumb wheel attached to the actuating shaft mechanism allows the actuating shaft mechanism to be rotated for further manipulating the laparoscopic tool.

    Abstract: A surgical apparatus for attaching two portions of cutaneous body tissue includes a pair of opposed elongated members having jaws which have tissue gripping members thereon at one end and which are movable toward each other to a closed position to engage two body tissue portions positioned within the members to move the body tissue portions into close approximation. Manually gripping systems of several alternative configurations are provided to actuate a mechanical transmission system to close the jaws. In a preferred embodiment, a fork is movable from a first position to a second position by manually operable devices to move the jaws toward each other. A pair of cam faces is located adjacent the opposite ends of the jaws. As the fork moves to its second position, the tines of the fork engage against the cam faces to move the opposed jaws to their closed position.
