The tube curves around a first tube support on the housing before passing through an opening in the bottom of the housing. The tube and any members inside the tube are cut off so they end inside the housing and are then covered by a housing cap. The base is fastened to a form for the concrete slab. The height of the housing relative to the base is adjusted to place the cap at the planned surface of the concrete slab. After the concrete slab is formed the cap is removed and the cut members inside the housing are connected to other members or devices.

An annular, foam plug extends between the inside of the skirt and the outer diameter of the tube. The plug has an undeformed inner diameter smaller than an outer diameter of the tube to create an interference fit around the tube and keep concrete out when the cap is on the end of the tube. The plug has a radial thickness of 0.3 to 1 inch, and has an axial length of 1 to 3 inches, with a bottom of the plug located at a bottom of the skirt. The cap is glued to the end of the tube for pressure testing. After the tube is entrained in concrete, the cap and plug are removed to leave a void in the concrete surface.

    Abstract: A pipeline pressure testing assembly has a fitting defining a flow path and having a port on a wall of the fitting into which a pressure testing device is removably inserted along a central axis of the port for pressure testing. The port is circular, larger than the interior diameter of the pipeline and located at or above the tube forming the flow path through the fitting. The fitting has a recess encircling at least part of the flow path and opening to the port to receive the testing and sealing devices. The recess is formed by two opposing walls inclined relative to the central axis of the recess. The recess walls at the top of the flow path by the port are spaced apart a distance as large as the pipe diameter for cleanout and inspection. A tubular valve stem screws into the testing device and has a rotary valve blocking flow through the tubular valve stem. Flow through the fitting is blocked with the rotary valve closed and the tubular valve stem in a first position.

Longitudinal channels separate the ridges. An extension tube has inward extending lugs passing along the channels to engage various ridges when rotated to fix the passage length. The extension engages the base bottom for corrugated deck supports. A cap with repositionable, locating filaments closes either tube.
