An example apparatus includes a workflow processor to identify a patient and determine a workflow for patient care, the workflow processor to divide a workflow into phases, each phase associating rules from a rules engine with at least one role, each phase including at least one activity specified by the rules, each activity involving at least one role and at least one associated task, alert, or suggestion for the at least one role specified by the rules, wherein the rules connect the activities of the phases of the workflow to an electronic medical record to automatically provide and receive data for a patient in each phase of the workflow. The example apparatus includes a phase monitor to monitor execution of the phases of the workflow and trigger at least one of the associated task, alert or suggestion.
