In certain specific embodiments, these dynamic and customizable medical forms may be automatically presented to users based on a predefined series of rules which allow multiple users having different roles in the clinical process to collaborate and contribute to a medical examination report, while at the same time maintaining an independent record of what was contributed and by whom it was contributed.
