    Abstract: A plurality of pilot hole impressing molds connected to a generally flat base structure constructed of resilient materials temporarily attached to the inner face of the temporary forms for the formation of the concrete structure. As concrete sets the forms and the pilot molds are removed exposing the pilot holes. Pilot mold extensions may be modified with metal caps temporarily attached. As concrete sets and forms and molds are removed, the metal caps remain permanently embedded in the concrete structure.
