A natural language processor (NLP) is used to determine NLP items in a selected document to present to a reviewing user. The NLP items in the selected document have content to review to correct for errors. A determination is made of an attribute value for the selected document. A determination is made of user interactions with the NLP items in the selected document having the determined attribute value. An order is determined to present the NLP items in the selected document to present to the reviewing user to correct based on the determined user interactions. The selected document, with the determined NLP items presented in the determined order for the reviewing user to review and correct, is transmitted to a computing device of the reviewing user.

The system determines a plurality of attributes relevant to each of the training cases. The system identifies a first attribute of the plurality of attributes as an issue, and a second attribute of the plurality attributes as a response to the issue. The system identifies a plurality of cohort members from the database of historical data, where each cohort member comprises cohort member attributes that match a subset of the plurality of attributes. The system analyzes the cohort member attributes of each of the plurality of cohort members to identify the most frequent responses to the issue. The system creates the training cases where each training case comprises the issue and the most frequent responses. The system then trains a machine based learning system using the training cases.

    Abstract: Embodiments provide a system and method for medical record error detection. Using a cognitive system having natural language processing capabilities, the error detection system can analyze a corpus of medical records, which can correspond to a particular patient, as well as a new medical record, and parse each into one or more record elements. By comparing the record elements, the error detection system can detect one or more potential errors resulting from a record element in the new medical record being inconsistent or contradictory with the record elements in the prior medical records, or ambiguous. The error detection system can output a list of candidate answers for correcting the one or more potential errors based upon a cognitive analysis of the corpus of medical records. The error detection system can further output a list of suggested therapies based on the selection of one or more of the candidate answers.
