    Abstract: A method and system for managing a chronic medical condition, such as chronic pulmonary obstructive disorder, is disclosed. The method includes generating a baseline score for a patient; recording an exacerbation severity for a plurality of symptoms; weighting the recorded exacerbation severities; determining an exacerbation score relative to the baseline score based on at least one of the recorded or weighted exacerbation severities; assigning the exacerbation score to a category; publishing the exacerbation score and assigned category for review by a physician or medical professional; and transmitting a treatment plan to the patient, the treatment plan being prescribed by a physician and based at least in part on the exacerbation score and/or assigned category. The system includes a computer that allows the patient to perform the steps of the method and a triage center for evaluating the published exacerbation scores and prescribing and communicating a treatment plan for the patient.

A baseline component is operable to establish a baseline indicative of a normal condition of the patient with the chronic condition. And a risk assessment component is operable to compare the condition of the patient as determined based on the variables to the baseline to determine a severity of the exacerbation of the chronic condition relative to the baseline.
