    Abstract: Systems, methods, apparatuses and computer program products are provided which utilize improved data storage and searching technique to rapidly identify records in a data collection. A database stores binary indexes which provide a condensed summary of characteristics for the records. An interface enables a search string to be generated using an interactive diagram that visually represents the search string. Selections are received via the graphical user interface to add nodes to the interactive diagram for defining the search string. The binary indexes are analyzed to quickly identify the stored records which satisfy the search string represented by the nodes added to the interactive diagram. A listing of the identified records is then made available for review and for use with various post-processing activities.

Information is stored for defining care channels corresponding to health categories which are utilized to classify patients based on health status and risk information. Patient information is retrieved from a plurality of data sources, and analyzed to detect care node flags that identify unfavorable health conditions and to assign a care channel to the patient. One or more care programs are assigned to the patient based, at least in part, on the assigned care channel and detected care node flags. The patient is transitioned to one or more care channels as the patient's health improves or degrades. A personalized health timeline is generated for the patient which summarizes the patient's medical history and other related information.

A workflow object is received that supplies information used to set particular attributes of the roles, the users, the business activities and the computer executable activities of the collaborative workflow process. Information and data objects are shared electronically among the users performing certain of the roles. At least one of the activities is automatically executed, such that the workflow is automatically managed.
