Abstract: A method of collecting patient information in an electronic system which eliminates the need to handwrite the patient information sheet, which exist in most doctor's offices. Patients complete their information on a secure website and the data is stored in either a remote or local database. The patient is issued two types of identification numbers or other type of authentication information. The first is used by the patient to update and view the information, and the second is provided to the doctor's office so they can download information about the patient without the need for patient information sheets.
