The health improvement program components are assembled into a plurality of configured health improvement program templates. One or more program templates of the plurality of configured health improvement program templates are configured to meet the requirements of a given population. The one or more health improvement program templates are deployed to a plurality of program participants within the population. a personalized health improvement program is delivered for each participant of the plurality of program participants within the population. Participant progress is monitored.
