In embodiments, the medical device visually presents patient data received from devices that more directly capture physiological data. The medical device is associated with a patient corresponding to the physiological data, and communicates the patient data to a centralized server for processing and forwarding to a database, which includes an electronic record that is specific to the patient. Then, the medical device may be dissociated from the patient.
