    Abstract: A set of forms having an OMR card for delivering packages is provided. Substantially all or part of information indicated on these forms can be input into a machine, without depending upon manual processing or with hardly changing the conventional process for manufacturing such forms or the method of filling them in.At least one of these forms is the OMR card. Of information to be written on the forms, information used for calculating the sending fee, such as the name and category of items, and the delivery substation code, is indicated by selectively marking characters, numerals, symbols or pictures, which have already been printed in a blank for marking these indicators. Location ID marks are printed on the card so that an OMR can read the marks.

    Abstract: In a small-sized data terminal apparatus with an image input function, a CCD image sensor inputs an image of an object for photoelectric conversion. A mirror is provided between the CCD image sensor and the object, for reflecting the image of the object to input it into the CCD image sensor. A waveform shaping circuit converts the output signal from the CCD image sensor into a digital signal. A timing pulse generating circuit controls the waveform shaping circuit and the CCD image sensor. A memory circuit stores data. A communication circuit transfers the data to external equipment. A control circuit processes the output signal of the communication circuit and the memory unit. A display circuit displays the output of the control signal. In one embodiment, the CCD sensor is a CCD area image sensor. In another embodiment, a timing detector is used in conjunction with a motor that turns the mirror.

    Abstract: An optical reader capable of optically reading a plurality of data comprises an object to be read having different coded information placed thereon, with each information having a unique control code, scan means for producing scanning light to be projected to said object, photo-electric conversion means for converting scattered light produced by the scan to an electrical signal, means for determining said control code and information by said electrical signal and means for sorting the information by said control code for storage, whereby different information is optically read while it is sorted by the control code unique to the respective information and stored.
