    Abstract: A method includes providing access to assets through communication channels, to a community of users associated with a medical condition, where access is provided by a system including a communication control that selectively enables user devices. Goals are established to modify a health-related outcome of the community of users over time and effect a particular treatment for the medical condition. The goals can include a time-in-range metric that defines an amount of time that a user of the community of users maintains the health-related outcome within a range bounded by a lower limit and an upper limit. The goals can include a total days of sensor data metric that defines an amount of time that the user wore a glucose sensor. Access to engagement rewards can be enabled based on the interactions and/or achievement of the goals to improve one or more health outcomes associated with the medical condition.
