  • Publication number: 20150095056
Abstract: Methods, systems, computer storage media, and graphical user interfaces for population health management are disclosed. The population health management system may allow for cross-continuum tracking and subsequent interactions with transactional systems, providers, and/or patients. The population health management system may include utilizing clinically relevant algorithms to populate registries to enable healthcare providers to better facilitate care for a population of patients. The population health management system may consolidate and provide comprehensive condition-specific and/or patient situation specific information that is accessible and updatable across venues. The population health management system may create cross venue antibiograms based on medications information and susceptibility results which can be filtered for selected demographics.
    Type: Application
    Filed: September 30, 2014
    Publication date: April 2, 2015
Inventors: Hugh H. Ryan, Chad Barrett Ruoff, Benjamin David Wilkerson

  • Publication number: 20140200915
Abstract: Readmission risks of patients admitted to a healthcare facility are determined using a generic readmission risk algorithm. The readmission risk assessment of patients may be based on portions of a patient's profile and may be performed before, during, and after their index admission. Based on the readmission risk assessment of patients, those patients that are at a greater risk for readmission may be identified. A readmission prevention worklist may be provided that identifies those patients and facilitates managing the risk of readmission for those patients.

  • Publication number: 20140180699
Abstract: Systems, methods, and user interfaces for providing dynamic risk stratification for clinical decision support are provided. Selections of patient types are received for patients. Assessments to utilize are determined in accordance with the patient types. The assessments are displayed to facilitate first clinicians assessing risk factors and contraindications for the patients. Information associated with the patients is received. Second clinicians may be alerted if risk factors for the patients are identified and the assessments have not been completed. Pharmacologic and/or mechanical prophylaxis is recommended and overall risk levels associated with the patients are indicated.
