    Abstract: A medical probe device for treatment of the prostate of a human male having a bladder with a base with a urethra formed by a urethral wall extending into the base of the bladder with the prostate having tissue surrounding the urethra near the base of the bladder with a catheter having a control end and a probe end and a passageway extending from the control end to the probe end along a longitudinal axis. A flexible stylet assembly is slidably mounted in the passageway in the catheter and has a distal extremity. The stylet assembly includes a conductive electrode and a sleeve of insulating material surrounding the conductive electrode and permitting a predetermined portion of the conductive electrode to be exposed.

    Abstract: An RF treatment apparatus provides multi-modality treatment for tumors and other desired tissue masses, and includes an RF indifferent electrode and an RF active electrode. The RF indifferent electrode has a compacted, non-deployed state, and an expanded deployed state. In the deployed state, the RF indifferent electrode forms a helical structure with four or less coils that surround an exterior of the tumor. The indifferent electrode becomes a microwave antenna when it is coupled to a microwave source. RF ablation energy is delivered to the tumor by the RF active electrode after it has been introduced into the tumor. Both electrodes are coupled to an RF energy source. The indifferent electrode is coupled to a microwave source. Either or both of the electrodes is hollow and include fluid distribution ports for the purpose of providing a chemotherapeutic agent to the tumor site. Ablation energy is supplied to the tumor by the active electrode.

    Abstract: A medical ablation device comprising a flexible RF electrode wire or tube, the terminal portion thereof extending through an insulating sleeve axially moveable thereon. The electrode and surrounding sleeve extend through a portion of a rigid tube having a distal end adapted to be inserted into the body of a patient. Tabs are attached to the proximal end of the sleeve and electrode for advancing them together from the terminal end of the tube, for retracting the sleeve from the distal end of the electrode to expose a predetermined area of electrode surface, and for retracting them together into the terminal end of the tube. The electrode can also be a hollow tube, the proximal end thereof being adapted to be connected to a suction source for aspiration of tissue adjacent its distal end or it can optionally contain a fiber optic, the end thereof being enclosed within the tube and closely adjacent its distal end.

    Abstract: A method and an apparatus for delivering controlled heat independently on separate channels to perform ablation to treat benign prosthetic hypertrophy or hyperplasia (BPH). The dual channel power system provides independent control of radio frequency energy to each of two stylets protruding from a catheter into a human prostate through the urethra. The energy is independently transferred directly into the respective tissue around the stylets in such a manner as to provide respective tissue ablation without damage to surrounding tissues. Automatic shut-off of both channels occurs when any one of a number of surrounding areas to include the urethra or surrounding mass or the adjacent organs exceeds predetermined safe temperature limits. Automatic shut off of the individual channels occurs when the temperature of the tissue proximal to the respective stylet exceeds respective predetermined values.

The stylet guide has a biopsy sample device or other tissue manifesting device at the distal end thereof to gather tissue or perform other operations at the point of the desired tissue such as emitting electromagnetic energy as ablative power to target tissues;.One embodiment of the biopsy sample stylet includes a hollow core sampler including a biopsy needle apparatus for gathering sample tissue. The hollow core sampler has a sharpened end for facilitating the penetration into and gathering of the sample tissue. A second embodiment of the biopsy sample stylet includes a harpoon-like sample stylet when the sharp tip thereof allows for easy entry into the target tissue.

The catheter assembly includes a stylet guide lumen communicating with the stylet port and a stylet positioned in said stylet guide lumen for longitudinal movement from the port through intervening tissue to a target tissue. The stylet can be an electrical conductor enclosed within a non-conductive layer, the electrical conductor being a radio frequency electrode. In normal action, the stylet and its surrounding sleeve are deployed out from the stylet port in the catheter into the desired target tissue. A sharpened tip on the stylet electrode facilitates the penetration of the intervening tissue, such as a urethra.

    Abstract: A method of this invention for treating body tissues containing cancerous cells or non-malignant tumors with RF ablation, alone or in combination with systemic or localized chemotherapy comprising introducing a stylet comprising an electrode surface and a sleeve longitudinally moveable thereon into the vicinity of the body tissues, retracting the sleeve from a portion of the electrode surface, and supplying RF power to the electrode surface sufficient to heat the tissue to a temperature of above about 45.degree. C. for a time to cause reduction of tissue mass in the vicinity of the electrode. The RF power supplied to the electrode surface is sufficient to effect a desiccated fluid diffusion barrier capsule surrounding the body tissue being treated. The stylet can include a hollow tube having fluid distribution ports therein, and the method can include the step of passing fluid through one or more distribution ports into the body tissue being treated.

The fiber optic channel means can include a guide port means for directing longitudinal movement of a fiber optic device with respect to the stylet guide means in a viewing zone and a flushing liquid channel in the stylet guide housing having an exit port positioned to direct flushing liquid issuing therefrom across the end of a fiber optic device when positioned in the viewing zone.

The procedure may be accomplished in a doctor's office without the need for hospitalization or surgery.

    Abstract: A dilatation balloon catheter with a non-removable guidewire is disclosed in which the guidewire is joined to the catheter tube through a distortable element, preferably a twistable tube. The element provides a strong fluid-tight connection between the guidewire and catheter tube and yet permits the guidewire to be rotated relative to the catheter tube over a wide range of rotation, with little torsional stress on either the catheter tube, the balloon or any other element of the catheter construction. In preferred embodiments, the catheter construction further includes a column support tube inside the balloon, surrounding a segment of the guidewire toward its distal end, to prevent collapse of the balloon along its longitudinal axis as the balloon is advanced into a stenosis. In these embodiments, the distortable element is joined to the catheter tube through the column support tube which thus serves as an intermediate linkage.
