In some embodiments, a valve prosthesis includes a central portion having an hourglass shape configured to pinch a native annulus in order to provide axial fixation of the valve prosthesis within a valve site. In some embodiments, a valve prosthesis includes a frame having an outflow end that is flared to provide a gap between an outflow end of the frame and an outflow end of prosthetic leaflets when the prosthetic leaflets are fully opened.

The prosthesis includes an inner support structure having a downstream section, and an upstream section, the upstream section having a cross-sectional area greater than the downstream section, the inner support structure being configured to be positioned at least partially on an atrial side of the native valve complex, and to apply an axial force directed toward a left ventricle. The prosthesis further includes an outer support structure having two or more engagement arms, the engagement arms being coupled to the inner support structure. The prosthesis is configured, upon implantation thereof, to clamp portions of leaflets of the native valve between the inner support structure and the engagement arms. An insertion device includes an outer tube and an inner pushing element. The outer tube constrains the hole closure device from expanding, during insertion of the hole closure device into the subject's body. The pushing element causes at least a portion of the hole closure device to self expand by pushing the portion out of a distal end of the outer tube, by the pushing element being advanced distally with respect to the outer tube. A safety element is couplable to the pushing element and is configured to prevent the hole closure device from expanding, by preventing distal advancement of the pushing element with respect to the outer tube, when the safety element is coupled to the pushing element.
