    Abstract: A method for providing care to individuals with dementia is provided. Initially, personal information about an individual with dementia, called person-centered care data, is developed. Next, the person-centered care data is distributed to caregivers, and care is provided to the individual based upon the person-centered care data. The person-centered care data may be developed through interaction with the individual and/or family members. Person-centered care data may also be developed through observation of the individual by caregivers, and may include individual biographical and preference information. Caregivers may be observed by supervisors or mentors when interacting with the individual, and provided feedback for performance improvement based upon the observations. Furthermore, goal-setting may take place between the supervisor or mentor and the caregiver to improve the ability of the caregiver to provide person-centered care.
