For example, the blank may be made 8½ by 11½ inches. The blank includes a front side for receiving printing thereon and at least one removable section in the card. The removable section is formed with a fold line that divides the section into two equal parts for subsequent folding onto each other. Both parts of the removable section are coated with an adhesive on the back side and a polymer sheet, for example, of polyester, is provided over the adhesive on one part of the removable section. A backing sheet, for example, of silicone coated paper is provided over the adhesive on the other part of the removable section. The blank may be provided with a plurality of removable sections so that multiple coupons may be formed from a single blank. A barrier coating is disposed on the second ply in facing relation to the repositionable glue to prevent adhesion of the repositionable glue to the second ply. Each label has one portion on the underside provided with a pressure-sensitive adhesive to allow the label to be temporarily attached, for example, to a page of a catalog. The adhesive-free portion of each label may be held in place in the form by ties or by adhesive on the opposed ply with a barrier coat therebetween. A form portion is secured to the label portion for processing through a laser printer or the like to receive information. In one embodiment, the form portion is removable from the label portion to permit the label portion to be secured along three lines of glue to a shipping carton to form a pocket therewith. The form portion is then folded and inserted into the pocket and the remainder of the label portion is then secured to the shipping carton to seal the pocket. In the embodiments, the form portion is folded under the label portion within the glue frame prior to securement of the label portion to a container.
