    Abstract: Computer program products, methods, systems, apparatus, and computing entities are provided for real time stratification of health-related data. In one embodiment, health-related data can be received in real time, after which rules corresponding to the health-related data can be identified. After identification, the rules can be applied to the health-related data to stratify patients into those who may be eligible for participation in and those who are ineligible for participation in one or more care pathways. Then, corresponding tasks can be created for the patients who are eligible for participation in the care pathways.
