Abstract: The method and system includes producing performance reports for payors that require performance evidence. The method and system collects a set of clinical data from the patient, combines the set of remote patient data with a set of EMR data, de-identifies this data, and produces a set of reports including compliance reports and outcomes reports, which can be tailored to a payor's needs, and further delivered to the payor. The method may be implemented as software to be executed on the system.

Abstract: The method and system includes efficiently routing remotely acquired patient data to both clinical and non-clinical care providers. The method and system collects a set of physiological data from the patient, accesses a set of routing rules from a routing database, and routes the set of physiological data to the appropriate care provider based on the routing rules. The method and system is configured to automatically route the set of physiological data, but may be configured for manual routing as well. The method may be embodied as software and executed on an appropriate system.

Abstract: The method and system includes producing a trajectory report to illustrate for the patient the benefit of complying with a prescribed treatment regimen. The method and system collects a set of physiological, self assessment, and compliance data from the patient, accesses a patient medical record database and a de-identified compliance and outcomes database, and calculates a clinical trajectory using a trajectory algorithm. The clinical trajectory is displayed for the patient on a graphical user interface, and illustrates for the patient the results of adhering to a prescribed treatment regimen compared to not adhering to the regimen. The method and system may be applied to any health condition that requires the patient to follow a treatment regimen.

Abstract: A patient monitoring system and method for associating acquired clinical data with a patient. The system can include a patient identification device that can be coupled to a patient, a data acquisition device that can be coupled to the patient to acquire clinical data, a patient monitor coupled to the medical acquisition device, a monitor identification device coupled to the patient monitor, and a code reader that can be used by a caregiver to read the patient identification device and the monitor identification device.
