    Abstract: A wristband device which is adapted to at least partially surround a person's wrist is described which includes a clamshell joinder and a tail portion. The clamshell portion comprises a pair of similarly sized panels and may be folded over onto itself to capture a length of a wristband to complete the encircling of a wrist. It may be provided on a page sized sheet, and die cut into a laminate ply of a two ply business form, along with a plurality of self adhering labels, in any of a number of configurations, to suit any particular application, as desired by a user.

    Abstract: A cushioned wristband includes a carrier having a multi-layered band portion and a strap attached such as by sewing or heat welding to the back of the band portion of the carrier. The band has a surface with loop material and the strap has a surface of hook material, the hook and loop surfaces being intended to secure the band in place as it is wrapped about a patient's appendage. The strap is appropriately sized to insert through one or both of two cinch slots of a self-laminating hang tag, and pressed against the patch of adhesive to secure it to the wristband. A fixative, such a an adhesive patch with peelable label is located about in the middle of the back of the strap and is used to closely position the hang tag along the length of the strap and ensure it remains flat against the strap/wristband as the wristband is worn. Alternate fixative arrangements are disclosed including hook and loop patches, etc, located at different positions on either or both of the hang tag and strap.

    Abstract: A preferably two ply, page sized business form particularly adapted for use in a medical or hospital environment includes a wristband that is die cut thereinto with a face stock area defined in the top or face stock ply and an overlying laminate portion defined in the underlying laminate ply with one or more cinch slots and a tab through which the strap end of the laminate portion is inserted for securing the wristband about a person's appendage. The business form preferably also includes a plurality of “special precaution” condition labels die cut into the laminate ply for selective separation therefrom so that one or more may be adhered to the face stock imaging area either before or after lamination to warn of a “special precaution” for the wristband wearer. The laminate portion, the label, and the strap end may be color coded to provide a visual indication of this special precaution status. The business form may be provided in sheetlet size, or full page size, one or more wristbands per form.

    Abstract: A business form such as a wristband or label includes an imaging area of face stock overprinted with a fluorescent ink for receiving a printed bar code to enhance the scannability of the bar code. The ink may be applied by overprinting to a thickness of between about 0.2 to about 0.5 mils, and even more preferably to a thickness of about 0.2 mils. The overprint ink is also print receptive allowing a bar code to be printed onto it with no deleterious effects. The imaging area of the wristband form may be overlaminated and the bar code scannability will still be enhanced. An alternative embodiment comprises a face stock manufactured to have a finished surface exhibiting a fluorescent effect similar to that achieved by overprinting through mixing in appropriate components as the face stock is made.

    Abstract: A business form particularly adapted for use during medical emergencies includes in a first embodiment a wristband/label assembly that is readily separable from a carrier, with the wristband including a single end for looping around a victim's appendage through a cinch and a tab carrying a plurality of labels with the wristband and each label having an identifying indicia such as a bar code printed thereon. The wristband bar code thus becomes associated with the victim and the labels are used to identify items associated with the patient such as his possessions, medical charts, medicines, etc. The wristband may be color coded so that as the medical personnel triage victims they are categorized by color as to their need for medical care, with the color coding thus being readily ascertainable by others as multiple victims are processed.

Multiple wristbands, or identification band blanks, are included on the same sheet to provide a form that is particularly suited for a pediatric admission. In an alternate embodiment, a panel is die cut into the transparent film layer so that upon separation of the wristband from the form, the panel remains adhered and it pulls away to form a window.
