    Abstract: A system and method are provided for managing patient care based on data received from a remote monitoring system. The system includes a data monitor for automatically evaluating the data received from the remote monitoring system and a scheduling module for receiving a data evaluation from the data monitor. The scheduling module includes tools for adjusting a patient schedule. Schedule adjustment actions can be automatically triggered by the received data evaluation. Typically, the data monitor will compare the received data with patient medical records to evaluate the received data received from the remote monitoring system to determine if the received data meets pre-set expectations. If the received data does not meet pre-set expectations, a scheduling action will be triggered. The system will automatically notify the patient of scheduling actions.
