The processors are further operable to determine a plurality of second health service providers that are each associated with at least one of the individuals, and determine a status associated with each of the individuals. The processors are further operable to generate and communicate for display an image based on the individuals, the second health service providers, and the statuses associated with the individuals. The image comprises a plurality of sections that each represent one of the individuals. Each section includes an indication in the section of the status associated with the one of the individuals. The image further comprises a plurality of groupings of one or more of the sections. Each grouping represents one of the second health service providers.
