Abstract: A healthcare management system for evaluating medical risk and health state of a consumer comprising a gateway configured to obtain clinical information and non-clinical information for the consumer, determine a composite healthcare index (HCI) for the consumer based on both the clinical information and non-clinical information, and share the composite HCI with a plurality of healthcare stakeholders that are authorized to receive the composite HCI, wherein the composite HCI is determined using a combination of a plurality of component HCI scores that are calculated using the clinical information and the non-clinical information.
